Initial Comments:
An unannounced, off-site follow-up survey, conducted on July 28, 2021, found that Centre Crossings Hospice had corrected the deficiencies cited under 42 CFR, Part 418, Subparts A, C, and D, Conditions of Participation: Hospice Care. The deficiencies were cited as a result of a Medicare recertification survey completed May 27, 2021.
